Elevate, Inc. was proud to have Tampa based Healthmap Solutions (Healthmap) CEO Joe Vattamattam present at our most recent board meeting. Healthmap was founded by Andrew Wall in 2007 with an initial emphasis on data management and analytics. Joe partnered with Andrew, joining the company in 2013.
Since then, Healthmap has added clinical algorithms and care services components to the offering to form a comprehensive population health management solution for its customers. Its flagship product focuses on the management of patients with chronic kidney disease and end-stage renal disease. Kidney disease is a $250 billion market impacting 37 million adult Americans and is the 9th leading cause of death, according to the National Kidney Foundation.
The founding principle of Healthmap is to reduce healthcare costs, improve the quality of care delivered and improve the overall experience for both the patient and the provider. Vattamattam went on to speak about the fragmentation of the healthcare & how the lack of proper lines of communication creates increased costs, an extra burden for the patient, and costs the provider increased time managing the administrative actions instead of utilizing that time to care for the patient.
Through the use of analytics and its Care Navigation team, Healthmap Solutions creates a plan for the patient that avoids unnecessary visits to the hospital while also relieving administrative costs for physicians. Healthmap’s ability to create the most efficient individualized care plan based on each patient’s healthcare-related circumstances has resulted in savings averaging north of $200 per patient per month for those patients eligible for its programs.
